Qualifications
- 5+ years of clinical experience in palliative care or complex chronic conditions required.
- 1-2 years of leadership or supervisory experience preferred.
- Strong communication and interpersonal skills necessary.
Responsibilities
- Provide leadership and support to nurse practitioners and physician assistants.
- Deliver comprehensive palliative care assessments and follow-up.
