Job summary
NJ Palliative Care Solutions is looking for an APP Team Lead to provide leadership and operational oversight for Nurse Practitioners and Physician Assistants in palliative care across Central New Jersey. This position involves mentoring, promoting clinical excellence, and supporting interdisciplinary collaboration to enhance patient-centered care for individuals with serious illnesses.
The ideal candidate should have extensive clinical experience, strong leadership qualities, and a commitment to improving quality of life. The role includes performance-based bonuses and requires flexibility with scheduling and travel throughout New Jersey.
Qualifications
- 5+ years of clinical experience in palliative care or complex chronic conditions required.
- 1-2 years of leadership or supervisory experience preferred.
- Strong communication and interpersonal skills necessary.
Responsibilities
- Provide leadership and support to nurse practitioners and physician assistants.
- Deliver comprehensive palliative care assessments and follow-up.
- Mentor and develop APP team members.
