Anpex Health is seeking a Nurse Practitioner or Physician Assistant to provide primary and palliative care with a focus on symptom management, goals-of-care planning, and comfort in seriously ill patients. This full-time position emphasizes team collaboration, patient-centered goals, and smooth care transitions.

The role includes leadership in care planning, family education, and coordination with an interdisciplinary team, with a structured monthly bonus and comprehensive benefits.

Qualifications

• Master’s (MSN) or Doctorate (DNP) Nurse Practitioner or Physician Assistant program completion.
• Experience in primary care, geriatrics, or post-acute care preferred.
• Palliative care training and certification provided.

Responsibilities

• Provide primary and palliative care focusing on symptom management, quality of life, and goal-concordant care.
• Conduct comprehensive history and physical exams for patients with complex illnesses.
• Lead and document goals-of-care discussions, including advance care planning and code status conversations.
• Facilitate completion and updates of advance directives and end-of-life care preferences.
• Manage pain and non-pain symptoms using pharmacologic and non-pharmacologic approaches.
• Review abnormal vitals and labs and escalate to attending physician as needed.
• Evaluate and manage new admissions ensuring alignment of treatment with patient goals.
• Collaborate with the interdisciplinary team weekly to review patient status and care plans.
• Identify patients appropriate for palliative or hospice referral and transitions of care.
