Job summary

Palliative Care Services seeks a dedicated Palliative Care Advanced Practice Provider (APP) to join our inpatient unit at Memorial Hospital, delivering expert, compassionate care focused on quality of life. You will work across care settings, leading symptom management, goals-of-care discussions, and interdisciplinary collaboration to tailor care plans to patient values.

Qualifications

• Current, unencumbered NP or PA licensure in applicable state.
• Board Certification in NP or PA specialty.
• Hospice and Palliative Care certification preferred or willingness to obtain within 12 months.
• Valid driver’s license and automobile insurance per policy.
• CPR Certification per state requirements.

Responsibilities

• Provide holistic assessments addressing physical, psychological, social, and spiritual needs.
• Guide goals-of-care conversations with patients and families.
• Create and update personalized care plans in collaboration with the team.
• Manage pain and symptoms with evidence-based strategies.
• Support safe transitions of care by reconciling medications and deprescribing when appropriate.
• Initiate timely hospice referrals and ensure smooth handoffs.
• Document assessments, care plans, and advance care planning in the EMR.
• Contribute to team meetings, case reviews, and quality improvement initiatives.
• Mentor colleagues and educate staff, referral sources, and community partners on palliative principles.
• Stay current with clinical guidelines, regulatory requirements, and best practices.
