Job summary

South Shore Health System seeks a Palliative Care Advanced Practice Provider (APP) to join a multidisciplinary team. The APP conducts initial and ongoing clinical assessments for patients with advanced illnesses referred for palliative care consultation, in inpatient, outpatient, or virtual settings.

The role includes modifying care plans with physicians, focusing on symptom management, goals of care, and coordination of care.

Qualifications

• Completion of Advanced Physician Assistant or Nurse Practitioner training program from an accredited institution.
• Licensure re-certifications as required (Massachusetts).
• ACLS certification (American Heart Association) required or included.

Responsibilities

• Provide initial and ongoing clinical assessments for patients with advanced and/or progressive conditions referred for palliative care.
• Modify plan of care in collaboration with attending and/or consulting physicians.
• Collaborate with interdisciplinary team to manage symptoms and support decision-making and goals of care.
