Job summary

Graham Healthcare Group is seeking a Palliative Administrative Specialist to join our team in Washington, DC. You will coordinate, schedule, and assign palliative patients to providers and serve as the primary contact for patients, families, and care teams.

This role emphasizes compassionate communication and data collection to support care plans. Qualifications include at least 2 years in Palliative, Hospice, or Home Care, a health-related bachelor’s degree preferred, and experience with

Qualifications

  • Minimum 2 years of experience in Palliative, Hospice, or Home Care.
  • Bachelor’s degree in a health-related field preferred.
  • Medical records experience preferred.
  • HCHB experience strongly preferred.
  • Proven data analysis skills with electronic health records.

Responsibilities

  • Coordinate, schedule, and assign palliative patients to providers.
  • Conduct follow-up calls to patients and families to collect data about response to the treatment plan.
  • Serve as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources.
  • Manage referrals, when needed, to appropriate agencies required to assist the client in achieving the goals identified in their Care Plan.
  • Assist patients in problem-solving potential issues related to the health care system, as well as financial or social barriers.
