Job summary

The Department of Veterans Affairs' Marion VA Medical Center in Illinois is seeking a Social Worker for the Patient Aligned Care Team (PACT) to provide clinical services in the Primary Care Clinic and Community Based Outpatient Clinics.

The Social Worker will assess and treat psychosocial needs, participate in an interdisciplinary team, and coordinate care across the continuum for Veterans and their families, including group education and case management.

Qualifications

• Assess and treat complex psychosocial needs of Veterans and their families.
• Establish and maintain therapeutic relationships with Veterans and their families.
• Provide psychosocial assessments to determine needs across the continuum of care.

Responsibilities

• Serve on the Patient Aligned Care Team (PACT) within primary care.
• Collaborate with interdisciplinary team to develop and implement treatment goals.
• Provide case management and group education for Veterans and families.
