Job summary

The U.S. Department of Veterans Affairs seeks a Social Worker to join the Social Work Service - Patient Aligned Care Teams (PACT) within the Veterans Health Care System of the Ozarks.

The incumbent will assess and treat Veterans, collaborate with an interdisciplinary team, and assist families with care coordination. Position is located physically in Branson, Missouri, with duties performed in a clinic/office setting.

Qualifications

• Must be licensed or certified by a state to independently practice social work at the MSW level.
• US Citizenship and English language proficiency required.

Responsibilities

• Complete assessments in collaboration with the interdisciplinary treatment team.
• Develop and implement treatment plans with Veterans and families.
• Educate Veterans and families about VA/community resources and referrals.
• Coordinate services with VA and community programs.
• Provide advocacy and support to ensure access to entitlements and services.
• Work Monday–Friday, 8:00am–4:30pm; after-hours may be required.
