Responsibilities

• Identify problems, strengths, coping skills and needed assistance; provide psychosocial support to Veterans with highly complex health and/or mental health problems.
• Perform comprehensive psychosocial assessments and write biopsychosocial assessments highlighting strengths and supports.
• Coordinate with VA staff and community services to meet Veterans' needs and establish referrals.
• Run psychosocial and psychoeducational groups for Veterans, caregivers and family members; act as care coordinator and case manager.
• Collaborate with PCMHI and Mental Health staff for warm handoffs when admission to inpatient programs is needed.
• Ensure safety plans (CSRE) and suicide risk assessments are completed as appropriate.
• Coordinate with interdisciplinary teams to ensure timely access to services and resources.
