The US Department of Veterans Affairs Northern Arizona VA Health Care System invites applications for a Social Worker in the Primary Care Teams (PACT). The role provides psychosocial services to adult Veterans, supports crisis response, and strengthens behavioral health integration within primary care.

Applicants must meet education, licensure, and English proficiency requirements; experience in integrated care and veteran-centered interventions is preferred.

Formación

• Knowledge of community resources and referral coordination.
• Ability to conduct psychosocial assessments and interventions.
• Knowledge of medical/mental health diagnoses and treatment planning.
• Skill in implementing diverse treatment modalities for individuals, families, and groups.

Responsabilidades

• Provide social work services to adult Veterans in PACT and support psychosocial crisis response.
• Assist in behavioral health integration within primary care settings.
• Coordinate care with interdisciplinary teams including nurses, PCPs, and behavioral health providers.
• Maintain clinical documentation and EMR records in CPRS.
• Meet licensure requirements and maintain Basic Life Support (BLS) certification.
