U.S. Department of Veterans Affairs seeks a social worker to deliver outpatient social work services within VA Ann Arbor Healthcare System’s PACT/GM & Specialty clinics.

Responsibilities include psychosocial assessments, treatment planning, case management, crisis intervention, advocacy, and medical care coordination. The role requires MSW from CSWE-accredited program and licensure/certification as per state requirements.

Formación

• Master's degree in social work from a CSWE-accredited program.
• Licensed or certified to independently practice social work (state requirement).
• Proficiency in English; ability to work with Veterans and families.

Responsabilidades

• Provide social work services to Veterans in PACT clinics.
• Perform psychosocial assessments and develop treatment plans.
• Coordinate care and refer to community resources.
• Collaborate with interdisciplinary treatment team.
