Job summary

The U.S. Department of Veterans Affairs is seeking a Social Worker to join the Patient Aligned Care Team (PACT) delivering longitudinal primary care to Veterans. The role emphasizes prevention, health promotion, and coordinated care.

The incumbent will assess for abuse, neglect, exploitation, and intimate partner violence; document findings; and collaborate with the MDT to support Veterans and their families, including referrals and case management.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Must be licensed or certified to independently practice social work.
• Experience assessing psychosocial functioning and formulating treatment plans.
• Ability to counsel individuals, families, and groups.

Responsibilities

• Assess psychosocial problems and their impact on health care.
• Complete thorough assessments to determine underlying causes and plan treatment.
• Coordinate with Veterans, families, and the multidisciplinary team for timely community placements.
• Provide consultation and education on community resources, VA benefits, and programs.
• Work schedule Monday–Friday, 7:30am–4:00pm; participate in performance-based duties.

Education
