Job summary

Veterans Health Administration is seeking a Registered Nurse Care Manager to oversee patient care for a high-risk panel within the PACT program. You will coordinate care, support transitions, and use data resources to guide population health initiatives.

Responsibilities include developing patient-driven care plans with interdisciplinary teams, providing health education and coaching to patients and families, and documenting interactions in the electronic record.

Qualifications

• Active RN license in any US state.
• Experience in care management for high-risk patients.
• Ability to coordinate with interdisciplinary teams and families.

Responsibilities

• Serve as primary contact for care management for an assigned patient panel.
• Develop and update patient-driven care plans with interdisciplinary teams.
• Educate, coach, and counsel patients and families on self-management and wellness.
• Collaborate with VA services and community resources to support transitions and comprehensive care.
• Monitor outcomes to identify care gaps and enhance patient wellness.
