The U.S. Department of Veterans Affairs Montevideo Community Based Outreach Clinic seeks a Registered Nurse Care Manager (RNCM) within the Patient Aligned Care Team (PACT).

The RNCM will provide comprehensive, coordinated nursing care to panels of veterans, with occasional supervision, in an outpatient primary care setting. Located in Montevideo, the RNCM will collaborate with the PACT and external services to promote health, manage chronic conditions, and facilitate smooth transitions of care

Qualifications

  • English language proficiency required.
  • Graduate of an approved nursing program and eligible for licensure in the United States.
  • Current full and unrestricted nursing license in a US state/territory or DC.
  • Foreign nursing graduates must obtain degree equivalency validation.
  • Temporary appointment options while pursuing licensure are possible.

Responsabilités

  • Manages care coordination for a designated panel of patients emphasizing health promotion and chronic care management.
  • Collaborates with PACT and external services to provide holistic, patient-driven care and timely access to resources.
  • Engages in patient and family education and advocates for patient needs to improve outcomes.
  • Leads coordination of care transitions and implements VA/DoD guidelines for chronic disease management.

Formation

Graduate of a school of professional nursing
Current state license to practice as a nurse
Nursing degree equivalency evaluation (foreign graduates)
