Job summary

The Registered Nurse (RN) in the Patient Aligned Care Team (PACT) at the U.S. Department of Veterans Affairs in Albuquerque coordinates patient-centered care by managing assessments, triage, chronic disease support, care planning and health education to improve access and outcomes.

Qualifications
