This full-time role, with no remote options, involves assisting with patient care, implementing care plans, and ensuring a welcoming environment for Veterans. The ideal candidate should have experience in Primary Care, an active Nursing License, strong computer skills, and a commitment to patient wellness.

Qualifications

• 1 year of experience in Primary Care or Chronic Disease Management.
• Must possess a current and active state Nursing License.
• BLS certification from the American Heart Association required.

Responsibilities

• Assist practitioners with patient care and assessments.
• Triage urgent/emergent patients for the care team.
• Provide patient education focused on self-management.
