Job summary

Aptive Resources, in partnership with Valor Healthcare, is hiring a Primary Care Physician (MD/DO) to deliver comprehensive primary care to Veterans within the Patient Aligned Care Team (PACT) model at the proposed Yreka, CA Community-Based Outpatient Clinic (CBOC), pending VA contract award.

Location is on-site at Yreka VA CBOC with a 0.05 FTE staffing plan and a full-time contract; clinic hours are typical, and duties include leadership of the PACT team and performance benchmarks for timely

Qualifications

• Education and training in Internal Medicine or Family Practice.
• Board certification or eligibility by ABMS or BOS in Internal Medicine and/or Family Practice.
• Prescriptive authority is required.

Responsibilities

• Provide care to assigned and unassigned patients per licensure and scope of practice.
• Ensure care plans include clinically indicated recommendations and services for PACT patients.
• Provide leadership to the PACT team and delegate appropriate care.
