Job summary

Aptive Resources is seeking a Midlevel Primary Care Provider (NP or PA) to deliver comprehensive primary care to Veterans at the Yreka VA CBOC, under the PACT model. This on-site role supports a 1.12 FTE staffing plan with 900 patient panels, working Monday–Friday during clinic hours.

The position requires licensure and credentials for California, VA credentialing, and a focus on team leadership, care coordination, and meeting access and encounter targets.

Qualifications

• Nurse Practitioner option requires: NLNAC/CCNE-accredited master's, NP certification, prescriptive authority, and active CA licensure.

Responsibilities
