Job summary

Aptive Resources, in partnership with Valor Healthcare, seeks a Physician Director to serve as medical director for the Yreka CBOC, overseeing primary care delivery under VA contract considerations. This on-site role is based at 807 S.

Main St., Yreka, CA 96097 and reports to the Aptive-Va partnership. Schedule is Monday–Friday, 7:30am–5:30pm with clinic operating hours and patient scheduling, on a full-time contract basis.

Responsibilities

• Oversee provision of primary care services at the site.
• Provide on-site supervision of mid-level providers per state requirements.
• Credentialing to function as a Primary Care Provider if proposed in a dual capacity.

Job Summary

Physician Director (PACT)

Overview: Aptive Resources, in partnership with Valor Healthcare, is hiring a Physician Director to serve as medical director for the proposed Yreka, CA Community-Based Outpatient Clinic (CBOC), overseeing primary care service delivery, pending VA contract award.

Location: On-Site | Yreka VA Community-Based Outpatient Clinic (CBOC), 807 S. Main St., Yreka, CA 96097

Schedule: Monday–Friday, 7:30am–5:30pm (Clinic Operating Hours); 8:00am–4:30pm Patient Schedule Hours

Employment Type: Full-Time Contract

Primary Responsibilities

• Oversee and remain responsible for the provision of covered primary care services to assigned and unassigned patients presenting for care at the site
• Provide on-site supervision of mid-level providers in accordance with applicable state requirements
• May also be credentialed to function as a Primary Care Provider (PCP) if proposed in a dual capacity (requires separate prescriptive authority authorization)
• Provide clinical leadership and ensure the site meets VA Patient Aligned Care Team (PACT) standards

Minimum Qualifications

• Evidence of education, training, and experience in Internal Medicine or Family Practice
• Board certified (or board eligible) by the ABMS or BOS in Internal Medicine and/or Family Practice
• Five (5) years of primary care experience without board certification/eligibility, or two (2) years with board certification/eligibility
• Must hold a current, full, active, and unrestricted license/certification (as applicable to the position) in California, or otherwise meet VA Qualification Standards for the position.
• Must successfully complete VA credentialing/privileging review and a background investigation (cleared through OPM via the VA Security and Investigations Center) prior to starting work; PIV credentialing required.
• Must complete required VA mandatory training and maintain current licensure/certification documentation annually.
