Job description

Summary

A relocation incentive may be offered to highly qualified candidates. The incumbent will serve as a Complex Patient Aligned Care Team (PACT) Physician located at the Daytona Beach VA Clinic in Daytona Beach, FL assigned to the Ambulatory Care Service. Assignments to other facilities within OVAHCS and travel to the site may be required to meet the needs of the service. This position is eligible for the Education Debt Reduction Program (EDRP).

Duties and Responsibilities
  • The Orlando VA Healthcare System (OVAHCS) is seeking an outstanding full-time Family Practice or Internal Medicine trained physician with recent experience in VA outpatient PACT panel management to join our Primary Care Clinic team as a Complex PACT (COM-PACT) Primary Care Provider.
  • Provide comprehensive gender‑specific care under the PACT model and intensive case management to a panel of patients with complex chronic medical conditions managed under PCMM guidelines to reduce recurrent ED visits and hospital readmissions.
  • Be attentive to patient education, preventive care, and work well in an interdisciplinary team to provide high quality primary care.
  • Manage chronic conditions, evaluate and treat inter‑current medical conditions.
  • Determine medical assessment and develop treatment plans according to accepted guidelines.
  • Collaborate with a PACT to provide accessible‑comprehensive‑coordinated‑continuous care to an assigned panel of patients.
  • Use a multi‑disciplinary team approach to improve patient care outcomes and prevent recurrent emergency department visits and frequent readmissions.
  • Carry a team cell phone from 7am‑9pm Monday‑Friday; the RN will handle the phone during business hours and the provider will hold it 7‑8am and 4:30‑9pm Monday‑Friday.
  • Perform disease prevention activities including gender and age‑specific screenings, mental health screenings, and health promotion activities.
  • Screen for, evaluate, and treat uncomplicated common mental health disorders, including depression and common addictions; refer to mental health providers as appropriate.
  • Provide clinical oversight and participate in peer review of other primary care providers through periodic chart reviews.
  • Act as surrogate for absent primary care providers when needed, including review of CPRS view alerts.
  • Attend primary care staff meetings, departmental meetings, and medical staff meetings regularly.
  • Document promptly in accordance with facility policy.
  • Complete all administrative work timely and address clinical reminders appropriately.
  • Maintain quality of care meeting professional standards.
  • Utilize all Virtual Care modalities as expected.
  • Provide timely follow‑up on results, including completion of view alerts.
  • Prescribe medication according to approved formulary and in accordance with Federal and State Law.
  • Consult other specialties as appropriate per service agreements.
  • Demonstrate flexibility in responding to clinic demands, adhere to personal and professional obligations, practice ethical standards, and sustain a harmonious work environment.

Work Schedule

Monday through Friday 8am‑4:30pm. Work schedules are determined and approved by the supervisor; the incumbent may be required to work various shifts on a permanent or temporary basis based on facility/patient care needs. The candidate must be willing to be detailed or temporarily assigned to other services and/or locations for operational needs or during regional or national emergencies.
