Job summary

The U.S. Department of Veterans Affairs Cheyenne VA Health Care System is seeking a PACT RN Care Manager to deliver coordinated, patient-centered nursing care within a team-based setting.

You will manage care for an assigned panel of patients, use evidence-based guidelines, and collaborate with the care team to promote wellness and effective transitions of care. Qualifications

• English language proficiency required for direct patient care as per 38 U.S.C. 7403(f).
• Graduate of a nursing program approved by ACEN or CCNE and licensure suitable for practice.

Responsibilities

• The PACT RNCM is self-directed in functioning as a team member providing comprehensive, coordinated nursing care to panels of patients.
• Focus on proactive, patient-centered care using nursing knowledge, evidence-based guidelines, and standardized nursing protocols.
• Manage care coordination for a designated patient panel, emphasizing health promotion, chronic care management, patient self-management, and overall wellness.
• Collaborate with PACT members, extended care support, and external services to develop and implement holistic, patient-driven care and to facilitate transitions and timely access to care.
