L’esprit Mental Health Services is seeking a compassionate PACT Generalist to join our multidisciplinary team in the Bozeman & Livingston area of Montana. This role delivers intensive, community-based support to adults with Severe and Disabling Mental Illness (SDMI) through the PACT model.

You will engage clients in their homes and communities, assist with daily living and treatment planning, and coordinate care with families and partners.

Qualifications

• High school diploma or equivalent required.
• Mental health experience preferred.
• Montana driver license and reliable transportation needed.

Responsabilités

• Provide person-centered client engagement and support in community settings.
• Assist clients with daily living skills, treatment planning, and recovery goals.
• Support clients in medical, psychiatric, employment, and community appointments.
• Help clients navigate housing, benefits, and community resources.
• Coordinate care with family, service providers, and community partners.
• Participate in treatment team meetings and collaborative care planning.
• Complete accurate and timely clinical documentation.
