Many Rivers Whole Health seeks a dedicated PACT Generalist to deliver recovery-oriented, community-based support for adults with severe mental illness in Great Falls, MT. The role emphasizes rehabilitation, advocacy, and coordination with a multidisciplinary team to help clients achieve independence.

You will work in homes and community settings, collaborating with clients to meet daily living goals and access housing, healthcare, and community resources. A robust benefits package is provided.

Qualificações

• High school diploma or GED required.
• Two years of experience in the mental health field preferred.
• Desire and ability to work with individuals with behavioral health challenges.
• Strong written communication, computer, and customer service skills.
• Valid Montana driver's license, personal vehicle and current vehicle insurance.
• Ability to travel throughout the community and transport clients as needed.
• Ability to pass Many Rivers Whole Health medication certification requirements.
• Ability to maintain effective working relationships with clients, families, providers, and community partners.
• Ability to maintain clear, accurate, and timely documentation.

Responsabilidades

• Assist clients in developing and achieving individualized recovery goals and personal recovery plans.
• Support clients with daily living skills such as hygiene, housekeeping, meal planning, and budgeting.
• Provide rehabilitation, coaching, and community support services in home and community settings.
• Deliver medication observation and support while maintaining agency medication certification requirements.
• Maintain an assigned client caseload and provide ongoing recovery-focused services.
• Coordinate with treatment team to develop and implement support plans.
• Assist clients in obtaining housing, healthcare, benefits, employment, and other resources.
• Provide advocacy to promote client independence and recovery.
• Participate in rotating on-call schedule including crisis response.
