Job summary

Veterans Health Administration, Department of Veterans Affairs seeks a Registered Nurse to provide age-appropriate, technically competent care in a primary care setting. You will coordinate care across the continuum, support patient education and self-management, and collaborate with an interdisciplinary team to improve outcomes.

The role requires an active RN license (US) and completion of an ACEN/CCNE-accredited nursing program.

Qualifications

• English proficiency required for direct patient care.
• Nursing education from ACEN/CCNE accredited programs preferred.
• Foreign-educated nurses require degree equivalency validation.

Responsibilities

• Provide age-appropriate, technically competent care in ambulatory or primary care settings.
• Coordinate care across the continuum and support patient education and self-management.
• Lead and collaborate with the interdisciplinary team to improve outcomes and satisfaction.
• Manage assigned patient panels, use registries and ensure timely access to care.
• Promote health promotion, education, and coaching on wellness and chronic care management.
• Demonstrate leadership to improve care across the department and facility.
