Qualifications

  • Bachelor’s degree in a mental health or related human-services field required in accordance with program contract requirements.
  • At least six months of experience in human services, behavioral health, community-based services, rehabilitation, or a related setting preferred.
  • Commitment to empowerment, recovery, personal growth, human rights, housing stability, and meaningful community integration for individuals served.
  • Ability to establish rapport and provide respectful, culturally responsive, person-centered support to individuals from varied backgrounds and with complex behavioral health needs.
  • Strong case-management, outreach, crisis-response, observation, problem-solving, and independent decision-making skills, with appropriate use of consultation and supervision.
  • Must obtain and maintain Medication Administration Program (MAP) certification within the introductory period of employment; training in the Sequential Intercept Model, LSI, and motivational interviewing is preferred and may be provided.
  • Valid driver’s license and a reliable vehicle available during all working hours required, along with the ability to meet BHN driving and transportation requirements.
  • Ability to meet the schedule and essential-staff expectations of a community-based program and the physical requirements of the role, including lifting, carrying, pushing, or pulling up to 40 pounds, with or without reasonable accommodation.

Responsibilities

  • Serve as the primary case manager for an assigned caseload, coordinating person-centered services that promote recovery, independent living, and community integration for individuals supported by the Program of Assertive Community Treatment (PACT).
  • Provide frequent outreach in individuals’ homes and community settings, build trusting relationships, and implement strategies that support individualized treatment-plan goals and objectives.
  • Coordinate and provide safe, timely transportation to medical appointments, treatment services, recreational activities, shopping, errands, and other community-based needs.
  • Support individuals in developing and maintaining social connections, strengthening relationships with family members and guardians, and participating in religious, recreational, and community groups of their choice.
  • Assist with activities of daily living, including food shopping, laundry, household tasks, money management, housing stability, and other skills that support greater independence.
  • Monitor individuals’ overall health and well-being and assist with medication treatment and administration in accordance with MAP requirements, agency policy, and the plan of care.
  • Provide crisis intervention and emergency support as needed, promptly communicating changes in risk, health, safety, housing, or behavioral health needs to the multidisciplinary team.
  • Collaborate with PACT clinicians, nurses, prescribers, rehabilitation staff, peer specialists, community providers, families, and other supports to deliver coordinated, integrated care.
  • Complete accurate progress notes by the end of each shift and submit incident reports, medication occurrence reports, treatment-plan documentation, and other time-sensitive records as required.
  • Maintain familiarity with PACT policies and procedures, provide coverage across PACT locations as needed, participate in required training and supervision, and perform other assigned duties.
