Job summary

Volunteers of America is seeking a Social Worker for the Senior Community Care of Colorado PACE program in Eckert, CO. This on-site role focuses on psychosocial assessments, care planning, and family education within a multidisciplinary team.

The position requires a Master’s in Social Work and active Colorado SW-C/LSW or LCSW licensure, CPR certification, and at least one year of elder care experience. Competitive salary with strong benefits.

Qualifications

• Master’s degree in social work or related field.
• Current CPR certification required.
• Valid driver’s license with insurance and transportation.
• Minimum 1 year experience with frail/elderly population.

Responsibilities

• Routinely assesses participant needs and coordinates with the IDT to implement care plans.
• Conducts initial enrollment assessments and six‑month reassessments.
• Participates in Interdisciplinary Team meetings and coordinates care across providers.
• Provides counseling and education to participants and families on aging, dementia, grief, and end‑of‑life planning.
• Facilitates admissions/discharges to contracted facilities and advocates for resources.
