Qualifications

• Master’s degree in Social Work required.
• LSW or LCSW licensure required.
• Experience with healthcare coordination and care planning preferred.

Responsibilities

• Provide social and emotional support to participants to help them achieve their goals.
• Participate in development and implementation of participant care plans.
• Document care provided promptly and accurately.
