Schedule: Monday- Friday 8:00-4:30pm
Position Summary:
ThePACESocial Worker provides and coordinates psychosocial services for participants and their familiesin thePACE Center, home, hospital, nursing home, or assisted living facility (ALF). The role also provides community-based case management services for programparticipants.
Position Qualifications:
- Is a graduate of amaster’sprogram in Social Work accredited by the Council of Social WorkEducation;and is licensed by the state of Kentucky as a Certified Social Worker (CSW) or a Licensed Clinical Social Worker (LCSW).
- A minimum of twoyears’social service experience working in a case management capacity.
- Must have at least one year of experience working with the frail and elderly.
- Strongdemonstratedcapabilities in networking with community resources, and strong knowledge of such resources.
- Strong communicationskills, including face-to-face, telephone, and computer skills.
- Strength in attention to detail and organization.
- Must be able to interact successfully with participants, families,and other agencies to present a positive and professional image.
- Knowledge and basic understanding of the interpersonaldynamicsoperative within the working relationships of an interdisciplinary healthcare team.
- Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload.
- Ability to workunsupervisedexercising a high degree of discretion & independent judgment.
- Ability to physically access any home, hospital, nursing home, and ALF (i.e., navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.).
