Bluegrass Care Navigators is seeking a PACE Social Worker to provide and coordinate psychosocial services for participants and families, in the PACE Center, at home, or in facilities as needed. This role supports community-based case management for program participants and works with an interdisciplinary healthcare team to tailor care plans.
The position requires an MSW from a CSWE-accredited program and state licensure (CSW or LCSW), with at least two years in case management and experience
As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Schedule: Monday- Friday 8:00-4:30pm
