Job summary

SCAN Health Plan seeks an organized Scheduler for its PACE program in Downtown Los Angeles. This 100% onsite role coordinates medical, therapy, and specialist appointments, ensuring care plans align with CMS and DHCS timelines.

The ideal candidate has 2+ years in medical scheduling, is fluent in English and Spanish, and can navigate EMR systems to keep schedules precise and compliant. Monday–Friday, 8am–5pm, with comprehensive benefits.

Qualifications

• Education: A High School Diploma or GED is required.
• Experience: At least 2 years in medical scheduling, healthcare administration, or related field.
• Elderly Care: At least 1 year of experience with frail elderly participants.
• Technical: Experience using EMR to keep schedules accurate.
• Communication: Strong interpersonal and problem-solving abilities.
• Organization: Highly organized with ability to manage multiple scheduling requests.
• Decision-Making: Comfortable making independent decisions in urgent situations.
• Language: Bilingual in English and Spanish.

Responsibilities

• Coordinate and Schedule Participant Appointments – Manage medical, dental, therapy, and specialty appointments per care plans and timelines.
• Facilitate External Appointments – Schedule visits with specialists, ensure referrals, authorizations, and transportation.
• Organize New Participant Orientation & Initial Assessments – Schedule orientation and multi-discipline assessments.
• Optimize Scheduling Across Care Settings – Act as quarterback to minimize conflicts and maximize efficiency.
• Collaborate with Key Teams – Coordinate with Transportation, Home Care, Medical Records, and Clinic teams.
• Liaison Between Key Departments – Primary contact among Senior Center Manager, Clinic Staff, Medical Records, Home Care, Transportation.
• Participant Communication & Follow-up – Confirm appointments and provide necessary details.
• Manage Scheduling Conflicts & Urgent Requests – Resolve conflicts and urgent needs to ensure continuity.
• Maintain Accurate Documentation – Update EMR/scheduling software per standards.
• Other duties as assigned – Adapt to evolving PACE program needs.
