Job summary

VALIR PACE LLC is seeking a dedicated RN to provide nursing care to PACE Participants, mainly in their homes, with possible care at the PACE center or other facilities. You will assess needs and implement plans of care under supervision and collaborate with a dynamic team.

Qualifications

• Active Oklahoma RN license without restrictions.
• Current CPR certification required.
• ACLS certification preferred.

Responsibilities

• Oversee nursing care for PACE Participants and ensure needs are met.
• Administer level of care assessments and nursing assessments.
• Deliver nursing care primarily in the participants’ homes; may occur at the PACE center or other facilities.

#PACE About the Role

Job Summary: Responsible for overseeing and providing nursing care to meet the needs of the PACE Participants and achieve goals set in the Plan of Care, as well as administer level of care assessments and nursing assessments. Nursing care and assessments will be delivered primarily in the participants home. They could also be delivered at the PACE center, or other in-patient facilities.
