Midland Care Connection seeks a Clinic Registered Nurse to deliver comprehensive, participant‑centered care to older adults within the PACE program. The PACE (Program of All‑Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Play a vital role in delivering comprehensive, participant‑centered care to older adults with complex medical needs. Working as part of an interdisciplinary team, the Clinic RN provides direct nursing care, coordinates treatments, monitors participants’ health, and helps prevent unnecessary hospitalizations while promoting the highest possible quality of life.
The Clinic RN performs nursing assessments, administers medications and treatments, manages chronic conditions, responds to acute health concerns, and assists providers during clinic visits. They educate participants and caregivers on disease management, medications, and healthy living, while closely monitoring changes in condition and communicating with physicians, therapists, social workers, and other members of the care team. The Clinic RN also maintains accurate clinical documentation, ensures compliance with regulatory standards, coordinates specialty appointments and follow‑up care, and advocates for participants to ensure their medical, emotional, and social needs are met. Through compassionate, proactive care, the Clinic RN helps participants remain as healthy, independent, and engaged as possible within the PACE program.
