Cherry Blossom PACE in Alexandria, VA is seeking a PRN Physical Therapist to evaluate and treat participants’ mobility and mobility-related assets for functional performance, promoting independence in self-care and daily tasks.

You will provide goal-directed, activity-oriented treatment, educate participants and families, and participate in care plan meetings with the therapy team as directed by the Therapy Director.

Qualifications

• Licensed Physical Therapist with active licensure.
• At least 1 year working with frail/elderly or approved training.
• Current CPR certification is required.

Responsibilities

• Evaluates and treats mobility and motor assets for functional performance.
• Provides goal-directed, activity-oriented treatment to promote independence.
• Educates participant, family or support system; participates in care plan meetings.

Ensures continuation of performance through effective education of the participant, family or support system. Provides adaptive methods and equipment as appropriate to enhance performance.
