Job summary

Cherry Blossom PACE in Alexandria, VA seeks a Licensed Physical Therapist to evaluate and treat participants' mobility and motor function, supporting independence in daily activities within a community-based program.

You will deliver goal-directed, activity-oriented therapy, educate participants and families, and collaborate with the care team in IDT and multidisciplinary meetings as directed by the Therapy Director.

Qualifications

• Licensed Physical Therapist.
• At least 1 year working with frail/elderly or approved training.
• Current CPR Certification.

Responsibilities

• Evaluate and treat participants' mobility and motor abilities for functional performance.
• Provide goal-directed, activity-oriented therapy to promote independence in self-care and daily life tasks.
• Educate participant, family or support system to maintain performance.
• Participate in family / Participant care plan meetings, IDT and various committees as determined by Therapy Director.
