Job summary

Center For Elders Independence is seeking an Intake Specialist I to navigate enrollment into the PACE program, coordinate with EDR, and drive census growth by enrolling participants and supporting retention. The IS I uses Salesforce to track leads and enrollments and acts as liaison among cross-functional teams.

The role requires communication of Participant Rights, eligibility verification, IDT scheduling, and travel within territory up to 60 percent.

Qualifications

• Bachelor’s degree in healthcare, business administration, or related field.
• Healthcare experience with Medicare/Medi-Cal reimbursement knowledge.
• Excellent customer service and service recovery skills.
• Strong case management and cross-functional teamwork abilities.
• Excellent written and oral communication skills.
• Proficient with Salesforce and standard office software.
• Ability to work in a high-stress environment and independently.
• Bilingual preferred: English/Spanish or English/ Cantonese or Mandarin Chinese or English/Vietnamese.

Responsibilities

• Coordinate and market the PACE program to potential participants and families.
• Verify eligibility per DHCS/CMS regulations.
• Explain Participant Rights and enrollment process; monitor progress.
• Schedule IDT assessments and LOC summaries for DHCS review.
• Coordinate enrollment meetings and review Participant Handbook and rights.
• Use CRM (Salesforce) to track enrollment cases and status.
• Support marketing outreach and events; assist with referrals and leads.
• Provide coverage at other PACE locations as assigned.
• Ensure compliance with state, federal, and company requirements.
