Job summary

Asian Health Services is seeking a Geriatric Services Manager to lead psychosocial assessments, care planning, counseling, referrals, and documentation across home, center, hospital, and community settings as part of the PACE IDT.

In this role you will supervise Geriatric Services Specialists, guide staffing and performance, and collaborate with the IDT to address medical, social, emotional, and spiritual needs while ensuring culturally responsive care.

Qualifications

• Master’s degree in Social Work required.
• Minimum 2 years health-related experience.
• Leadership and/or management experience required.
• At least 1 year of experience working with a frail elderly population; if not, complete PACE training.
• Bilingual Cantonese and/or Mandarin preferred; culturally and linguistically competent services.
• Strong knowledge of community resources, care coordination, psychosocial assessment, and documentation standards.
• Ability to provide respectful, participant-centered service to older adults.

Responsibilities

• Lead IDT assessments, care planning, and reassessments for PACE participants.
• Oversee delivery of medical social services, care management, counseling, referrals, and participant advocacy.
• Supervise Geriatric Services Specialists; oversee staffing, scheduling, performance, and team accountability.
• Partner with the IDT to develop and implement care plans addressing medical, social, emotional, and spiritual needs.
• Monitor participant changes and communicate health and psychosocial status to the IDT.
• Maintain social work documentation in the PACE EHR per regulations and policies.
• Maintain quarterly progress records in the participant’s EHR.
• Coordinate 24-hour care delivery, discharge planning, home and facility visits, and follow-ups.
• Serve as liaison among participants, families, caregivers, and community providers.
• Educate participants, caregivers, and staff on aging, dementia, grief, and PACE model.
