Job summary

Asian Health Services seeks a PACE Physical Therapist to oversee the PT program for SpringLight Health participants. You will evaluate and treat participants, develop individualized care plans, and collaborate with the IDT to address mobility and functional needs, including caregiver and staff education.

Home visits may be required to deliver education and assessments. The role requires a California PT license, bilingual Cantonese or Mandarin abilities, and experience with EHR systems within a

Qualifications

Must hold Master’s or Doctorate in Physical Therapy.
Current California Physical Therapist license in good standing.
BLS Certification and immunizations up-to-date.

Responsibilities

Evaluate, assess and treat PACE participants with musculoskeletal conditions and mobility impairments.
Develop and implement individualized PT care plans in collaboration with the IDT.
Attend IDT meetings and report on mobility status and functional changes.
Provide PT education to participants, caregivers and staff; may include home visits.
