Job summary

InnovAge Holding Corp. is seeking an experienced Outreach Specialist to drive sales for the PACE program across assigned territories. You will educate referral sources, manage outreach, and work toward enrolling eligible participants through a structured, field-based strategy.

You will use Salesforce CRM to track activities, build relationships with healthcare professionals, and expand InnovAge PACE referrals while meeting monthly targets and contributing to census growth.

Qualifications

• Experience in post-acute, senior, and managed care settings.
• Ability to enroll 8 eligible participants per month.
• Self-starter with results-driven approach.
• Willingness to educate referrals about PACE services.

Responsibilities

• Execute Marketing/Sales Plan and exceed enrollment quotas.
• Develop and execute territory sales strategy for PACE.
• Prospect new account opportunities and educate referral sources.
• Maintain CRM records of all sales activities and follow-up.
• Provide ongoing education about InnovAge PACE services to referral sources.
