Job summary

BoldAge PACE is seeking an Outreach Specialist to drive program growth through strategic outreach, relationship development, and high-quality prospect engagement. This role leads the front line of Outreach & Engagement, identifying, qualifying, and converting prospective participants into enrollments while educating the community about the PACE model of care.

Qualifications

• High school diploma required; Bachelor’s degree preferred.
• Minimum of 3 years in community outreach, relationship-based sales, or business development.
• Knowledge of PACE, Medicare/Medicaid eligibility, and local senior service systems preferred.
• Proficiency in CRM systems and Microsoft Office Suite.
• Strong written, verbal, and public speaking skills.

Responsibilities

• Develop and execute a strategic outreach plan aligned with growth goals.
• Identify, qualify, and cultivate high-quality referrals from healthcare providers and community organizations.
• Conduct consultative conversations with prospects and caregivers to understand needs and align to PACE services.
• Maintain timely documentation of outreach, referral, and prospect activities in the CRM.
• Build and maintain relationships with referral partners and participate in community events.
