Job summary

Trinity Health Mid-Atlantic is hiring a Community Liaison to grow awareness and referrals for our PACE program in Maryland. The role focuses on building referral partnerships with hospitals, physicians, and senior organizations, educating the community, and guiding eligible seniors to care and support services.

Candidates should have 2+ years in healthcare sales or outreach, strong networking and presentation skills, and the ability to manage a territory independently.

Qualifications

• 2+ years in healthcare sales, outreach, business development, or community engagement.
• Experience in senior care, home care, hospice, or PACE.
• Strong networking and relationship-building skills.
• Excellent communication and presentation skills.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, rehab centers, senior centers, and community organizations
• Educate referral sources, families, and community partners on program benefits
• Generate qualified referrals and support enrollment growth
• Conduct presentations, community events, and outreach activities
• Maintain territory plans and referral pipelines
• Collaborate with enrollment, clinical, and operations teams to ensure smooth participant transitions
