Trinity Health PACE is seeking a Community Liaison in Montgomery County, MD to connect with referral sources and promote our PACE program. This full-time role requires coordinating with community partners, delivering presentations, and using a CRM to manage referrals.

The position offers a Monday–Friday schedule, a driver’s license, and opportunities to advance within the organization. Bilingual skills are a plus.

Qualificações

• Bachelor’s degree preferred, with at least two years of sales or marketing experience.
• Experience partnering with senior community resources such as hospitals, senior living communities, and home care companies is preferred.
• Valid driver’s license required.

Responsabilidades

• Develop relationships with referral sources and healthcare professionals to educate them about PACE and secure referrals.
• Collaborate with the team to strengthen community presence, identify sales opportunities, and build referrals.
• Implement sales strategies to meet growth objectives with management support.
• Provide in-services and presentations to the community about program benefits.
• Utilize CRM to track all referral information accurately.
