Job summary

Navajo Health Foundation - Sage Memorial Hospital, Inc. is seeking a P4 Program Team Lead to manage grant-funded initiatives under the Produce Prescription Pilot Program.

You will coordinate team efforts, report on activity, and collaborate with community partners to improve access to fruits, vegetables, and related wellness programs. Qualifications

• Bachelor’s degree in public health, health education, community health, human services, communications, or a closely related field.
• At least two (2) years of experience coordinating community health programs and/or grant funded activities.
• Ability to pass Employee Health Program requirements and background check.

Responsibilities

• Coordinate and assist team members with deliveries of F&V boxes and gift cards to program recipients.
• Provide office coverage and be available for participants via dedicated P4 main number.
• Work with participants to resolve voucher or card issues.
• Collaborate with community partners to report and resolve voucher/card issues.
• Submit monthly grant/activity reports and analyze data.
