Job summary

Navajo Health Foundation in Ganado, AZ seeks a P4 Program Team Lead to manage and implement grant-funded initiatives under the Produce Prescription Pilot Program. You will coordinate daily workload, assist team members, and support the design and execution of community health activities that enhance wellness and access to care.

This role involves reporting, partner collaboration, and program development across wellness, nutrition, diabetes, and other public health areas.

Qualifications

• Bachelor’s degree in public health or related field and at least two years coordinating community health programs or grant-funded activities.
• Must pass Employee Health Program requirements and background investigation.
• Knowledge of health issues impacting Navajo Nation communities and related interventions.

Responsibilities

• Coordinate and assist team deliveries of F&V boxes and gift cards to program recipients.
• Provide office coverage and be available for participants via dedicated P4 contact numbers.
• Work with participants to resolve voucher or card issues and report program activity status.
• Plan, coordinate, and implement grant-related program activities per strategic plan.
• Develop community partnerships and engage stakeholders in program development.
