Qualifications

• OD degree from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD/DO) with valid state ophthalmology license.
• General Liability and Professional Liability insurance certificates.
• ME/VA license agreements with Gumberg Asset Management Company.

Responsibilities

• Launch optometry services in Sam’s Club and lead patient care for eye health.
• Establish your days, hours, and patient schedule; control fee schedules.
• Maintain ownership of exam/professional fees; participate in equipment upgrades.
• Opt into administrative and/or opt-tech support services where allowed by state law.
