Qualifications

• OD degree from an accredited optometry school.
• Valid state optometry license.
• MD or DO with ophthalmology license in the state.
• General/liability or professional liability insurance certificate.
• ME/VA license agreements handled via Gumberg Asset Management Company.

Responsibilities

• Launch your services in Sam's Club as an Independent Optometrist leasing space inside our stores.
• Establish your days of practice and hours of operation.
• Control your patient schedule.
• Set your fee schedule and retain 100% of exam/professional fees.

Education

OD Doctor of Optometry from an accredited optometry school

Valid state optometry license

MD/DO ophthalmologist with license

General Liability Certificate/Professional Liability Insurance Certificate
