Qualifications

• OD degree from an accredited optometry program
• Valid state optometry license
• Ophthalmologist license (MD/DO) from an accredited school

• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 ME & VA license agreements with Gumberg Asset Management Company
