Qualifications

• Bachelor’s Degree in Social Work, Psychology, or related field.

Responsibilities

• Provide community support including transportation to programs, medical, benefits or employment appointments.
• Deliver case management services including support with medical benefits and coordination with therapists and treatment teams.
• Maintain Medicaid billing and accurate service documentation.
• Participate in supervision meetings and multidisciplinary treatment team collaborations.
• Support daily living skills and medication management for individuals served.
