Qualifications

• Bachelor's degree in a human service field or similar.
• Valid Driver's License.
• PACE Certification.

Responsibilities

• Provide individualized supports in residential and community settings.
• Follow and implement plans, goals, and treatment strategies.
• Apply Safety Care de-escalation and crisis procedures as trained.
• Record and maintain required data and documentation.
• Engage with people supported to promote autonomy and skills.
• Foster positive relationships with people supported and staff.
• Complete paperwork promptly and accurately.
• Consult with professionals on goals and techniques.
• Support transportation to appointments and activities.
