Qualifications

• English literacy required to follow care plans and document services.
• MAP certification training will be provided within 6 months of employment.
• CPR/First Aid training available within 3 months of employment.

Responsibilities

• Assist individuals with daily living activities and personal hygiene.
• Transport individuals to appointments and community activities in wheelchair van.
• Complete ISP and daily logs, track behavioral data.
• Administer medications as required and maintain documentation.
• Provide companionship, mentorship and supervise safety in the home.
