Job summary

Christian Living Communities offers a PRN overnight position (6:45 PM–7:15 AM) every Saturday as a Personal Care Partner II/QMAP role. The role focuses on meeting daily personal care needs, supervising medications under a licensed nurse, and ensuring accurate MAR documentation.

Qualifications

• Medication certification required (QMAP-A/L, Med Tech, Med Aide, Nurse Delegation).
• Must be able to read, write and speak English.
• One year of elder care experience preferred.

Responsibilities

• Attends to the personal care needs of residents in a timely manner and provides assistance with ADLs as requested or directed.
• Provides personal care services (bathing, dressing, meals, laundry) with attention to individual resident needs.
• Anticipates and reports resident care needs to supervisor.
• Responds to family members as appropriate.
• Maintains open communication with supervisor/peers about resident issues.
• Monitors safety and well-being of residents.
• Reports suspicion of abuse, neglect, or misappropriation of resources as required by regulation.
• Monitors and documents resident status and condition as directed.
• Assists with prescribed daily medication routines and notifies RN/LPN of concerns.
• Ensures timely documentation including refusals of medication/treatment.
• Restocks medication carts and storage areas.
• Participates in monthly staff meetings.
• Other duties as assigned.
