Job summary

Grace Pointe in Greeley, CO is seeking PRN QMAPs to support residents in Assisted Living/Memory Care. The role involves administering medications as ordered by the attending physician, under supervision, and following care plans to meet resident needs.

Qualifications

• QMAP certification required.
• Experience in assisted living/memory care preferred.
• Ability to follow care plans and assist with activities of daily living.

Responsibilities

• Answer resident call lights and assist with needs.
• Follow service plan to meet resident needs and assist with ADLs.
• Escort residents to activities and dining, serve meals and beverages as needed.
• Deliver meals to residents' apartments when appropriate.
• Provide laundry/linen service per service plan.
• Report changes in resident status to Charge Nurse per procedures.
• Administer and document medications as ordered by physician.
